Donate Stock
If you choose to donate stock to Green Hill Therapy, you can either use your brokerage firm to assist in the transaction or simply contact our brokerage firm.
You can donate stock in one of the following two ways:
1. You can instruct your broker to deliver your stock electronically. Simply call Shirley Cochran at (502) 244-8011 and she will provide you with the following information:
- Green Hill Therapy's brokerage account
- Green Hill Therapy's broker
- The address
- The account number
- Green Hill Therapy's Tax ID number
- The DTC number
2. Send the UNENDORSED stock certificate(s) and stock power(s) by registered mail to:
